WAIVER 1: PHOTO & VIDEO RELEASE (MINORS)

■ PHOTO & VIDEO RELEASE — REQUIRED

I am the parent or legal guardian of the child being registered, and I hereby grant Riptide Golf, LLC, its owners, employees, contractors, and representatives permission to photograph and/or record my child while participating in Riptide Golf, LLC activities, programs, lessons, camps, clinics, events, or use of facilities.

I understand that these photos, videos, or recordings may be used for legitimate business purposes, including but not limited to social media, marketing and promotional materials, advertising and informational content, and print or digital media related to Riptide Golf, LLC.

I acknowledge and agree that my child’s name will not be used without additional permission, no compensation will be provided, Riptide Golf, LLC may edit, crop, or format media as needed provided it is used respectfully and appropriately, and such images will not be used in any way that is knowingly harmful, misleading, or inappropriate.

I understand that this authorization is voluntary and may be revoked in writing at any time; however, revocation will not apply to materials already created or published prior to receipt of written notice. By checking this box, I certify that I am the parent or legal guardian of the child listed and that I have the legal authority to grant this permission.

WAIVER 2: LIABILITY RELEASE & ASSUMPTION OF RISK (MINORS)

■ ASSUMPTION OF RISK & RELEASE OF LIABILITY — REQUIRED

I am the parent or legal guardian of the child being registered. I acknowledge that participation in golf, sports, recreation, training activities, and the use of equipment and facilities at Riptide Golf, LLC involves inherent risks, including but not limited to physical injury, accidents, falls, contact with equipment or other participants, weather conditions, and unforeseen hazards.

On behalf of myself and my child, I voluntarily assume all risks, known and unknown, associated with my child’s participation, including the risk of serious injury or death. I hereby release, waive, discharge, and hold harmless Riptide Golf, LLC, its owners, members, officers, employees, instructors, coaches, volunteers, contractors, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to personal injury, illness, property damage, or death that may occur as a result of my child’s participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I further agree that my child is physically capable of participating, I will disclose any medical conditions, allergies, or limitations that may affect participation, Riptide Golf, LLC is not responsible for lost, stolen, or damaged personal property, and supervision may be limited during certain activities and transitions.

In the event of an emergency, I authorize Riptide Golf, LLC to seek medical treatment for my child if I cannot be reached, and I agree to be financially responsible for any such treatment.

This agreement shall be governed by and construed in accordance with the laws of the States of Florida and Massachusetts, without regard to conflict of law principles. By checking this box, I acknowledge that I have read and understand this waiver and that I am giving up substantial legal rights on behalf of myself and my child.
